That would be a masochist. People who like pain inflicted upon themselves. Sadists like to inflict pain on others.
As to the expansion itself hurting... I'd bet that the saline filling itself isn't painful but of course the needle is. Skin stretches alot and it would take a fair amount of stretching for breast skin/tissue to really start to ache, depending on of course the persons build and how much tissue they had in that area. The lady in the video has enough excess skin that 2 cup sizes wouldn't do anything in the pain realm i'm betting.
I remember having emergency stitches on my thumb from cutting down to the knuckle accidentally. By the time I got seen I had already stopped the copious bloodflow from direct pressure. When they did the stitches I asked to watch being curious. They injected some painkiller stuff directly into my thumb and I was shocked to see my thumb swell up to about three times its size. My thumb was HUGE. Of course the novicane was there to stop the stiches from hurting. Took a few hours for my body to absorb all of the painkiller and return back to normal. The dulling effect wore off after 30 min or so and the only pain I had was from the cut itself being aggravated.
