this is a doujin game japanese made (thought it is english) and is same system as mario but has a picture of your character and the character is a witch girl.
it has 5 Age type little kid (the one you start), kid after get 1 mushroom, teen (break blocks) ,your age (throws fireballs) and adult (same as star i guess)
hmm my opinion the adult form is kinda... weird? doesnt look the same person thought.
ok just confirmed that there is no other form but after you finish you play as her rival that AR'ed her At the end you get your revenge AR her so the "extra" part is playing with her as a child and she won't AP also
