I'm a fairly experienced DM and I still don't see the need for the rules you're trying to put in place, especially since the rules are going to be varied based on what the character is transforming into. If the person grows claws, grant them a claw attack. If they get sleeker, give them a dexterity bonus. Let's take it further.
If they're turning into a dragon, give them draconic eyes and a breath weapon of the appropriate element. If they're turning into a displacer beast, give them displacement and two extra appendages. It really sounds to me like this would be a case by case basis. As for sheerly creative ideas, I quite like an earlier idea in which you curse otherwise normal equipment. In a dungeon crawl involving nature worshipers, equipment looted from them and their thralls should not necessarily be deemed safe.
Can you give me more information? What are the villains of this campaign shooting for? What are they like? What are their methods? That sort of stuff. Xanderfox was more or less spot on; just because it's not in a book or spelled out anywhere, there's no reason you can't do it.
