Thoughts on combat:
The dynamics allowed by real-time combat wouldn't mix too well with fappability, even with one-handed controls.
A few notes on what I'd like to do for MY system (take what you wish from this, all tentative):
Strictly turn-based, Two player-controlled characters vs. multiple enemies per battle. Options would be modular in the sense you can add or remove them on the fly, but rather self-explanatory and simple. Should probably go through no more than three menus to choose an action (Select Mammary Magic, Select Oppai Missile, Select one enemy).
The two player-controlled characters are a boy and a girl. No need for a complex custom character system. In hentai games, it always leads to someone wanting to play as a futa, and most BE artists I know don't enjoy futa. Having a boy and girl allows for cute tender moments and various sexy shenanigans between the two.
