Sorry to be the bearer of bad news but it's not. It's just the transformation of Bullet turning into Anya completing and then him passing out from the transformation. Also that isn't breast expansion it's Age Progression since you see her get older. Don't worry I wasn't confused aswell and I had someone point it out to me and double checked with the Doujin.
