I found on Wikipedia an H-Game related to Cthulu Mythos (which was why I found it) called Saya No Uta. This contains spoilers, so...

It says that one of the three endings has a character who's "slimy and has tentacles" release a spore which transforms the entire human population into such creatures.

Seems like a massive long-shot as to whether there will be process into "tentacle monsters" as apparently the actual appearance of the girl who releases the spores is never shown, and I cannot find any relevant videos or pictures online. However, I thought you guys might be interested to know.

I Have a copy but was never able to get it to run so after a while I gave up, Nor do I know how to rip the CGs. I'll try to get it working one more and let you know what I find.

Didn't really see anything in the CG files for the game. Nice art though.

The only transformation it shows is when she grows a wing-like flower bloom, run the title through Google Images and you'll find pics of that rather easily. She never transforms from monster to human, she is the monster through-out the game, her human form is simply the cause of the protagonists warped perception of reality: Normal becomes monstrous, monstrous becomes normal.

The spore mutagen thing is all episode/post-script I believe, and is not shown visually.

I found a summary with some pictures and I had gathered the whole "She's a monster throughout" part, but it's a massive shame that that wasn't shown in detail.

It is, after all, the perfect Tentacle Transformation scenario. Pity.

Found one in Sexy Magical Girl Ai epasode 3 one of the school grils becomes posessed and grows tentacles. About 9min into the epasode.
