Maga Tsuki [AP]
Maga Tsuki is an ecchi comedic harem manga in which the protagonist meets a goddess and his soul gets ejected from his body and it cannot return. The only way for the main character to remain alive is to have his soul enter the girl, goddess, or spirit who holds the greatest amount of affection for him and have them within physical contact with the main character's body (such as holding hands).
The AP part happens in chapter 46 in which Izuna, a 100 year old fox spirit whose appearance has been that of a high school girl, has received enough love from the main character to permanently transform her into a buxom adult (sorry, it's another poof transformation). Both images are from the same chapter. While the AP occurs late into the manga, I recommend the series as a form of leisure reading. Izuna pre-transformation:  Izuna post-transormation:
