That's from Makai Kishi Ingrid, as it says in the title, specifically the first volume, out of at least four. Looks like there's a lot of tentacles all around, but based on how lazy that transformation was I wouldn't put much faith in there being any decent transforming going on, especially since hentai budgets usually dip drastically the longer a series goes on.
