I'm remaking my MOS to include a size called Overboard, to serve as an ultimate limit (I'll reserve it for jokes and situations rather than casual character design), to make Xtreme look more managable, and also because I needed to make one more size for a specific character design. I've only showed this to Sutibaru though, and I think he agrees that it's all right.

But yeah, I'd say the largest acceptable size is about the size of small beanbags, not quite body-size. But for a "normal" character design, I'd say Massive is a casual enough maximum (but I'd be sticking around Normal-Huge). Xtreme and Overboard are the limit and limit-break, respectively. I tend to find it adorable when the girl can hug herself, and holds/carries her breasts in the frames of her arms (like in my display pic. Those are Massive-size btw).

Same. I'm trying to incorporate a little bit of reasonable sag, but not so much that they look sloppy and droopy. I like pillow-esque fluffiness and shape.

Really, as long as they aren't as big as her body, it's all good ;D

Considering the list britty showed, I would probably stop at N. However, I used the K model often in my artwork. One question, though: I have trouble reading the conversion scale. What would K's size be if transfered to real life size?

As for real people, I'd probably stop before it looks fake.

I'd personally stop at P cup but like most I prefer the process then the actual bra cup size of the finished scene.

Depends on the situation, really. I prefer it to stop before being completely immobile is an issue, but even then it varies. Some situations call for more, some for much less.

Even in fantasy, E is pushing it for me. Why dream about that which would just be awkward to play with....?

I've seen plenty of girls who look great and 'normal' (used here just to say looking like she won't tip over) at EE, I've seen Ds that look too big in reality depending on the girl's body frame.

for me theres no such thing. i usually find it difficult to find new stories involving breasts larger than the woman. which is a shame cause i love those stories.
