I could be wrong, but I think this is from the live action movie Perfect Blue. (not to be confused with the anime movie by the same name).

Could be zdarkness, I never was able to find the movie to watch it. I know in the anime movie of the same name, there was no such gender change. But both center around a plot about an actress and her would be stalker and how mind twisting it gets...

I actually have this Japanese live action movie. It is called PERFECT BLUE. Same title as the anime, but different story. It's not a hallucination. A young man ends up transforming into the teen pop star that he idolizes for no apparent reason. He just does.

Ah, right. In the anime film it's just a guy who thinks he's the pop star.
And then it turns out there's also a girl who thinks she's the pop star. I think... I haven't seen it in a while, and I've never actually seen the film in its entirety.
