From memory I think it's a game caled Kuro No Utahime by Clockup.
That image is the bad ending for one of the characters and the only one in which you see her corrupted like that.
Clockup have done a few corruption/hypno games before. Dyogrammaton being another good one.

I usually tend to see CGs on the 2chan EC threads and try to find the games myself, this is one of the few times I got stumped. Skimming through ExHentai's GameCG entries over the weekend got me nowhere.
