The protagonist's thought-to-be-dead sister appears before him, but it is actually a monster's tail in disguise.

Decent process is found, she disfigures into something monstruous, but sadly she is only the tail of the actual monster, who is simply hidden beneath the leaves.

There is another character which seems to mutate into a Komodo dragon in the same episode, but we only see a couple of bits of the process (face and left claw).

It kinda acts like a symbiote suit, but unlike those, the body can better be seen changing.

One of them is some sort of Underworld Goddess returning from a her monster to a human form. 5:50

At 8:56 the Komodo Dragon character transforms for the last time into her monster form.

There are also several Human to panther and viceversa transformations throughout episodes 11 and 12 for those interested.
