Yes, but by using novicane you wouldn't feel anything localized to the injection site, I.E. your thumb. So if it ached from the swelling induced by the novicane, you'd never know.

Right but once the novicane wore off it didn't hurt except where the cut was nor did it bother me as the swollen area went back to normal. Also when the injection happened my thumb swelled up instantly and novicane doesn't activate instantly. The doctor had to wait a short bit for it to take effect, which was fast, just not instantly. . That being said you are correct on the skin aching once it took effect as once it took effect I'd not know it, till the painkiller wore off.

Now I'm pretty sure they could always add a slight numbing drip if they really wanted to if it did cause enough pain to bother someone who was having her boobs inflated that way

I tried to hunt down one of the threads in the BE Archive forum concerning this just so I could link it a while ago in case anyone wanted more information on the process, but I just wasn't having much luck on it. But if it was being done professionally by a plastic surgeon, say because brides want to look good for their weddings and such, then one would have to wonder why they wouldn't either cut a little novicane into the saline or inject some before they started the drips. Skin stretches, but still... I imagine the pressure would lead to at least a mild ache for the first few hours at least. It's kind of strange that you don't see more about this, but I suppose it's because of the possible complications if something goes wrong and the expense of the doctor's time for growth that's not going to last more than a few days.

Yes, it would be the same. In fact, the saline would do the same thing no matter where you injected it. Some people do it to put temporary 'horns' in their foreheads to look purposely demonic

As SkruLuce said, it would work regardless of sex. You may have noticed that the breasts didn't quite have a natural look to them and that's going to be present either way. And if that's not quite what you meant, then I actually do recall a thread in the BEA's real world forum that mentioned using this in the scrotum with some success.
