It is a boy given the shot and he becomes a girl (mostly). You can tell by looking at the first image you posted.

When characters are drawn with such a feminine face, I tend to think of them as female, but guess I was wrong in this case.

Credit goes to SoyleB over @ masterbloodfer's forum who uploaded another manga with a BE scene. This time it's an elf couple scene where the wife cannot seem to produce any milk for her child. To remedy that, they see a doctor which injects the wife and increases her breast and causes her to lactate.

I got my hands on Comic Unreal volume 3, and it does indeed have at least two separate stories with BE, but the quality is relatively low. I'll wait a bit to see if I can get better scans before posting.

No, I got that same vibe...that clown from spawn weirds me out
