Unfortunately this kind of research isn't really good for finding causes or explanations. We can speculate until the cows come home. Even something as safe as saying the communal nature of nursing home living lends itself to increased incidence of severe infection is an unbacked statement unless we have research with it.
If I had to make such a statement to answer your question, I'd say that the nursing home population is indeed sicker and older than the non-nursing home patients, so that's why they die more often after surgeries. So what that's really saying is that it's not the nursing home status that matters, it's age and whatever other diseases the patients walk into the OR with. HOWEVER, in one of the procedures (a colon surgery) I found that nursing home status WAS indeed a factor all in itself influencing death rates. Non-nursing home patients had a 2.0% death rate after this surgery while nursing home patients had a mortality of 12.6%. Six times more. It's tough to try to guess why that is.
